FRIDAY, JUNE 1^st – 1:00 p.m. – 5:00 p.m.

MONDAY, JUNE 4^th – 6:00 p.m. – 9:00 p.m.

WEDNESDAY, JUNE 6^th – 8:00 a.m. – 12:00 noon

THIS BLUE PHYSICAL FORM MUST BE FILLED OUT AND TURNED IN TO THE NURSE’S OFFICE OR THE TRAINING ROOM BY

TUESDAY, MAY 29^th   

YOU MUST SCHEDULE AN APPOINTMENT TO RECEIVE A PHYSICAL! 

If you miss your physical appointment, you are responsible for scheduling one with your private physician, documented on the blue physical form, and returned to the ATHLETIC TRAINER prior to your first practice.
